A parent or guardian signs these three forms. Bring them on a Wednesday, or scan and email them to gmonroe@ucdavis.edu.

Mentors

These clear you to work one on one with students. You’re welcome at group sessions while they’re in progress.

We submit the request for you. UC Davis Universal Background Screening then emails you directly. If you signed up more than a week ago and haven’t heard anything, email Grey.
