Students & families
A parent or guardian signs these four before the student’s first session. Print them, sign them, and bring them to your first Wednesday. If it’s easier, scan and email them to gmonroe@ucdavis.edu.
1. Waiver of Liability (Form GW17)
The standard UC participation waiver. Available in English and Spanish.
2. Consent to Record
Photo and media release. This is what lets us share a student’s photo, name, or project on the website, at the showcase, and on social media. Declining is completely fine and does not affect participation.
3. Authorization to Consent to Treatment
Lets us get emergency medical care if a parent or guardian can’t be reached right away.
4. Emergency Information & Contacts
Who we call in an emergency, plus any medical or allergy information we should know. We carry this on every off-campus trip.
5. National EnvironMentors registration (online)
Registering with GCSE is what makes a student officially part of the national EnvironMentors program, and it’s what makes them eligible for the national science fair. It takes a few minutes and asks for parent or guardian contact information. Select University of California, Davis as your chapter.
Does your project involve hands-on lab work? Only some do, and many are field-based. If yours does, a few extra safety steps apply and your mentor walks you through every one. Field trips get their own permission slip closer to each trip.

Code of Conduct with Minors
Every UC Davis mentor signs this before working with students, alongside the background check and trainings covered on the FAQ. Print it, sign it, and hand it to Grey or Erwan.
